布托啡诺复合依托咪酯脂肪乳用于无痛人工流产术的观察

目的探讨布托啡诺复合依托咪酯脂肪乳用于无痛人工流产术的麻醉效果。方法拟行无痛人工流产术的妊娠妇女120例，随机分成4组，每组30例。A组：异丙酚复合芬太尼组；B组：异丙酚复合布托啡诺组；C组：依托咪酯脂肪乳复合芬太尼组；D组：依托咪酯脂肪乳复合布托啡诺组。待患者睫毛反射消失，不能应答时开始手术。观察不良反应发生例数，用药前后生命体征的变化，记录麻醉效果。结果各组患者均能较好的完成手术。术后VAS评分，A、C两组及B、D两组差异无统计学意义（P〉0．05）；不良反应的发生，与A组比较，其他各组呼吸抑制的发生率明显降低（P〈0．05）；与B组比较，C、D两组恶心、呕吐发生率减少（P〈0．05）；与C组比较，D组肌阵挛发生率明显减少。结论布托啡诺复合依托咪酯脂肪乳能较好的适用于无痛人工流产术。

An Investigation of Analgesia Artificial Abortion by Butorphanol Combined with Etomidate

Objectlve To investigate the anesthetic effect of analgesia artificial abortion by butor- phanol combined with etomidate.Methe~s A total of 120 early pregnant women were randomly divided into four groups. Group A （n= 30） was treated with fentanyl combined with propofol, Group B （ n = 30） with butorphanol combined with propofol, Group C（n = 30） with fentanyl combined with etomidate, and Group D （n = 30） with butorphanol combined with etomidate. The operations began when eyelash reflex disappeared and the patient could not answer any ques- tions. The systolic blood pressure （SBP）,diastolic blood pressure（DBP）, heart rate（HR）, fre- quency of respiration（RR）, blood oxygen saturation （SpO2） were recorded at different time points, and the visual analogue scales（VAS） was used to judge the pain from uterine contrac- tion after the operation,the adverse effect, recovery time and leaving hospital time were observed respectively.Results All the patients in each groups had a better state to finish their operation. Compared with Group A and C, the differences of postoperative VAS score were not statistically significant in Group B and D（P〉0.05） ; with regard to the occurrence of adverse reactions, compared with Group A,the incidence of respiratory depression in other groups was significantly reduced （P〈0.05） ; compared with that of Group B, Group C and D had a lower incidence of nausea and vomiting （P〈0.05） ; Compared with that of Group C, Group D significantly reduced the rate of myoclonus.Conclusien It has been proved to be effective to conduct analgesia artificial abortion by using butorphanol combined with etomidate.
