非酒精性脂肪性肝病患者血清溶血卵磷脂水平的研究

摘    要:目的探讨非酒精性脂肪性肝病(NAFLD)患者血清溶血卵磷脂(LPC)水平变化以及其与非酒精性脂肪性肝病的关系。方法选取符合纳入标准的非酒精性脂肪性肝病和正常对照人群各33例。收集并分析两组人群人口学特征以及临床资料,萃取分析血清所含脂质,比较组间血清溶血卵磷脂的相对含量,并对溶血卵磷脂进行多因素非条件Logistic回归分析。结果 NAFLD组LPC 18∶2、LPC 20∶5相对含量均低于正常对照组[(23.99±4.86)μmol/L vs(28.50±6.91)μmol/L,(3.88±0.89)μmol/L vs(4.68±1.33)μmol/L],差异有统计学意义(P0.01)。多因素Logistic回归分析发现,校正年龄、性别混杂因素,并纳入腰臀比、谷丙转氨酶、甘油三酯以及空腹血糖危险因素后,LPC 18∶2、LPC 20∶5的OR值分别为0.86、0.47,表明血清溶血卵磷脂与NAFLD呈负相关。结论血清溶血卵磷脂与NAFLD呈负相关,LPC可能成为有意义的生物标志物,用于诊断并预测NAFLD的发生。

Abstract:Objective To investigate the change of serum lysophosphatidylcholine (LPC) in nonalcoholic fatty liver disease (NAFLD) by lipid analysis and the relationship of LPCs and nonalcoholic fatty liver disease. Methods 33 patients with NAFLD and 33 matched disease-free individuals were analyzed. Anthropometric and clinical data were collected. 300 Ixl serum of 66 subjects were obtained for lipid profiling by UPLC-MS. Student's t test, X2 test and multifaetor logistic regression analysis were used. Results Subjects with NAFLD showed significantly lower serum LPCs (LPC 18 : 2, LPC 20 : 5 ) than those without NAFLD [ ( 23.99±4.86 ) μmol/L vs (28.50±6.91 ) μmol/L, ( 3.88 ±0.89 ) μmol/L vs (4.68 ± 1.33 ) μmol/L, P〈0.01 )]. Multifactor logistic regression analysis showed that serum LPCs were negatively correlated with NAFLD. Conclusion Lipid analysis found a decrease in serum LPC level in NAFLD group. And LPC could be used as biomarker to predict and diagnose NAFLD.
