摘    要:目的统计分析辽西地区某三甲医院重症监护室(ICU)患者血培养标本的病原菌分布特点及耐药性,为临床合理用药提供依据。方法回顾性分析2008年1月至2012年12月某三甲医院ICU 974例标本的血培养及药敏试验结果。血培养采用BD-BACTEC9120全自动血培养仪及专用血培养瓶,药敏试验采用Microscan-walkaway 96全自动细菌鉴定仪。结果 974份血培养标本中分离出病原菌167株,阳性率为17.1%。药敏结果显示,革兰阳性菌对万古霉素敏感,革兰阴性菌对复方新诺明和头孢他定较敏感。结论 ICU患者血培养分离的病原菌种类多,耐药性差异大。对血培养分离病原菌和耐药性进行早期监测可为临床合理用药提供参考。

Abstract:Objective To analyze the distribution and drug resistance of pathogens isolated from blood culture specimens of patients in ICU and guide clinical rational use of drug. Methods Retrospectively analyzed 974 cases blood culture specimens and the result of susceptibility test from 2008 to 2009. The BD-BACTEC9120 automatic hemoculture system was used to identify bacteria. The Microscan-walkaway 96 automatic microbiological analysis system was used to test drug sensitivity.Results Blood culture was performed for a total of 974 cases and 167 cases were positive. The positive rate was 17.1%. The drug susceptibility test showed that the gram-negative bacteria were most susceptible to SMZ-TMP and ceftazidime, and the gram-positive bacteria were most susceptible to vancomycin. Conclusion The pathogens isolated from blood specimens in ICU were widely distributed and have great differences in the drug resistance. It is necessary to early monitor the blood culture and drug resistance, which can provide important evidence for rational use of drug.
