2004-2005年我院老年人肺部感染抗生素应用分析

目的 探讨老年人肺部感染治疗中抗生素使用的现状及其合理性。方法 按感染菌种不同分组，考察抗生素使用的合理性，并结合药物经济学分析方法，设计合理可行的临床个体化给药方案。结果 抗生素应用合理占43．42％，基本合理占19．30％，不合理占37．28％；药物经济学评价结果革兰氏阳性菌组青霉素的成本／效果（CEA）值最低，革兰氏阴性菌组左氧氟沙星合并头孢呋辛的CEA值最低，衣／支原体组左氧氟沙星的CEA值最低，真菌组氟康唑的CEA值最低；临床应用结果显示，CEA值最低的治疗方案切实可行。结论 老年人肺部感染抗生素应用不合理的比例较高，应引起临床的高度重视。

Study of Antibiotic Application in Treating Pulmonary Infection of Old People in Qingdao Municipal Hospital from 2004 to 2005

Objective By investigating the aged patients with pulmonary infection in Qingdao Municipal Hospital, to study the rationality of antibiotic use. Methods To divide the cases into the different groups according to the pathogenic bacterium and to survey the change of the parameters before and after the treatment. Results Rational using of antibiotics was 43.42%, basic rational using was 19. 30% and irrational using was 37.28% respectively. The drug with the lowest value of CEA in different groups was penicillin, levofloxacin hydrochloride and fluconazole respectively. The best project which was established in studying to treat the pulmonary infection of old people was feasible. Conclusion The proportion of irrational using of antibiotic is higher which should be recognized highly by the clinician.
