| 摘 要： | 目的:观察采用抗生素降阶梯治疗对慢性阻塞性肺疾病合并重症肺炎患者的疗效。方法:68例慢性阻塞性肺疾病合并重症肺炎患者随机分为治疗组和对照组,治疗组34例采用抗生素降阶梯疗法,对照组34例采用常规抗生素治疗,比较两组疗效。结果:抗生素降阶梯治疗的治疗组病死率、机械通气时间、ICU住院时间、总住院时间、C反应蛋白、治疗后患者动脉血二氧化碳分压(PaCO2)均低于传统抗生素治疗的对照组,差异均有统计学意义(P<0.05),动脉血氧分压(PaO2)、动脉血氧饱和度(SaO2)、呼吸衰竭指数(RFI=PaO2/F iO2)均高于传统抗生素治疗的对照组,差异均有统计学意义(P<0.05)。结论:抗生素降阶梯疗法对慢性阻塞性肺疾病合并重症肺炎具有良好的治疗效果。
