松果体区乳头状肿瘤临床病理分析

目的:探讨松果体区乳头状肿瘤的临床病理学特征、免疫表型、诊断和鉴别诊断。方法:对1例松果体区乳头状肿瘤进行HE和免疫组化染色,并复习相关文献。结果:组织学上肿瘤富于细胞,瘤细胞排列成片状和乳头状。乳头中央为宽阔的纤维血管轴心,表面被覆一层或几层肿瘤细胞,部分血管壁玻璃样变性。可见灶性坏死。瘤细胞呈上皮样,细胞界限清楚,胞质空淡或淡嗜伊红,核圆形、卵圆形,染色质颗粒状,核仁小,核分裂像高达10个/10HPF。免疫组化标记,肿瘤细胞弥漫表达CK、NSE,CK18局灶阳性,Ki-67增殖指数为10%,而GFAP、CgA、Syn、S-100蛋白阴性。结论:松果体区乳头状肿瘤是一种罕见的颅内肿瘤,组织学最显著特征是肿瘤呈上皮样生长模式,光镜下不易与乳头状室管膜瘤、脉络丛肿瘤及转移性乳头状癌等鉴别,应用免疫组化标记可有助于鉴别诊断。

Clinicopathologic analysis of papillary tumor in the pineal region
