摘    要:目的探讨急性胰腺炎(AP)时环氧化酶-2(Cox-2)表达与腺泡细胞凋亡的关系。方法 SD大鼠30只,随机分为假手术组(A组)、急性水肿性胰腺炎组(B组)和急性出血坏死性胰腺炎组(C组)。采用逆行胰胆管穿刺注射法复制急性水肿性与出血坏死性胰腺炎模型。检测造模后12 h各组的血清淀粉酶活性、腹水量、胰腺组织大体病理改变及评分、镜下病理改变与评分、腺泡细胞凋亡指数(AI)及Cox-2蛋白表达情况。结果①C组血清淀粉酶活性、腹水量、大体及镜下病理评分、Cox-2阳性率显著高于B组(P<0.01),AI显著低于B组(P<0.01)。②B、C组中AI与大体及镜下病理评分均呈显著负相关关系(P<0.05)。③B、C组中Cox-2表达与AI均呈显著负相关关系(P<0.01)。结论 AP时腺泡细胞凋亡是机体的一种自我保护机制,Cox-2过度表达可能通过抑制腺泡细胞凋亡加重胰腺病变。

Abstract:【Objective】 To explore the relationship between Cox-2 expression and apoptosis of acinar cells in rats with acute pancreatitis.【Methods】 Thirty SD rats were randomly divided into three groups: group A(shame operation group),group B(acute edematous pancreatitis group) and group C(acute hemorrhagic necrotic group).Two kinds of pancreatitis model were induced by retrogradely injection of sodium deoxycholate.After 12 hours,the serum amylase activity,ascites,apoptotic index(AI) of acinar cells and expression of Cox-2 were observed,the general and histological changes of pancreas were evaluated by the corresponding score system.【Results】 The serum amylase activity,ascites volume,general and histological pathology score,positive rate of Cox-2 in group C were significantly higher than B(P <0.01),AI lower than B(P <0.01).In group B and C,AI were negatively correlated with general and histological pathology score(P <0.05).In group B and C,the expressions of Cox-2 were negatively correlated with AI(P <0.01).【Conclusions】 The apoptosis of acinar cells is a self-protection mechanism of individuals with AP,Cox-2 inhibiting the apoptosis of acinar cells and deteriorate pancreatic disease.
