临床药师在病区合理用药中的药学监护

目的:探讨临床药师在病区合理用药中的作用。方法:临床药师通过参与临床查房,发现不规范用药隐患与不合理用药典型病例,与临床医护人员共同分析、改进。结果:临床药师在病区的规范用药、医嘱用药、药物警戒、药学服务等方面均发挥了重要作用。结论:临床药师通过参与查房缩短了与临床的距离,同时也为患者的安全、合理用药提供了保障。

Rational use of clinical pharmacist in ward in the pharmaceutical care

Objective: To investigate the role of the clinical pharmacist in rational drug use of ward.Methods: Clinical pharmacist through participation in clinical rounds find that non-standard drugs hidden and irrational drug use typical case,analysis and improve drug use with clinical staff.Results: The clinical pharmacist in the ward's standard medication,ordered by your doctor,pharmacovigilance,pharmacy services,has played an important role.Conclusion: Clinical pharmacist by participating in clinical rounds shortens the distance with clinical,and has provided a guarantee for patient safety,rational use of drugs.
