应用支气管镜肺减容的动物(犬)实验研究

目的探讨应用组织封闭剂经支气管镜行肺减容术的可行性。方法成年杂种犬6只,麻醉后经口插支气管镜入亚段支气管,在支气管扩张球囊的协助下,注入组织封闭剂;采用影像学(CT,术后1h、4周、8周)和组织病理学方法(术后8周)检测靶段肺组织情况。结果所有操作过程顺利,未发现技术操作上的困难,没有动物死亡,术后无明显并发症;术后8周CT检查示6只杂种犬靶段肺组织均呈密度增高致密影,未见炎性病变影像,病理检查肉眼见肺表面瘢痕组织形成,切片示阻塞部位失去正常肺组织结构,形成纤维致密结构。结论应用组织封闭剂经支气管镜行肺减容手术操作容易,创伤小,并发症少,可用于肺减容治疗。

Animal study on bronchoscopic lung volume reduction

Objective To investigate the possibility of bronchoscopic lung volume reduction with biocompatible fibrin-base glue.Methods Six dogs were used in this experiment,bronchoscope was inserted to the subsegmental bronchi through mouth and was infused biocompatible fibrin-base glue with the help of CRE sacculus dilation catheter,target lung was detected by roentgenographic examination and histopathological methods at 1h,4w,8w and 8w postoperation respectively.Results Procedure of operation was successful without technic difficulty,no animal was dead,and no complications were observed.High density tissue was found at 8w postoperation.Inflammatory pathological changes were not observed.Under microscope,scar tissue was seen.Conclusion Operative procedure through bronchoscopic lung volume reduction with biocompatible fibrin-base glue is easy,trauma is little,complications are few,and so this method will be used in lung volume reduction.
