老年2型糖尿病患者定量超声骨密度及其与中医证候的相关性分析

目的 探讨老年2型糖尿病(DM)患者的骨密度改变及老年2型糖尿病性骨质疏松的证候特征.方法 对136名老年2型DM患者进行超声骨密度测定,114例健康老年人作为正常对照组,并对老年2型糖尿病性骨质疏松进行中医证候评定.结果 与正常对照组相比,70岁以上年龄段的患者超声振幅衰减平均值(BUA)显著降低;60～64岁女性2型DM患者的BUA显著降低.将BUA与中医证候积分进行相关分析,老年2型糖尿病性骨质疏松组在阴虚证、肾精亏虚证和瘀血阻络证中呈现负性相关关系.结论 老年女性2型糖尿病患者的骨密度变化与年龄相匹配的健康人群比较无明显变化,而70岁以上的男性患者骨密度较老年健康人有所降低.老年2型糖尿病性骨质疏松与阴虚证、肾精亏虚证和瘀血阻络证具有密切关系,从而验证了它们在老年2型糖尿病性骨质疏松中的客观存在.

Correlation between quantitative ultrasonic BMD and TCM syndrome manifestations in senile patients with type 2 diabetes mellitus

Objective To investigate the changes of bone mineral density(BMD) and syndrome characteristics of osteoporosis in senile patients with type 2 diabetes mellitus.Methods The ultrasonic examinations on BMD were given to 136 senile patients with type 2 diabetes mellitus,and other 114 healthy aged people were included in the normal control group.The evaluation of TCM syndrome manifestations was accomplished in the osteoporosis due to type 2 diabetes mellitus.Results Compared to the normal control group the indexes of BUA decreased significantly in the patients over 70 years old and female patients of 60-64 years old.The results of analysis on BUA and TCM syndrome integrals showed that the osteoporosis due to type 2 diabetes mellitus had negative relation to the syndromes of yin-deficiency,kidney-essence deficiency and blood stasis obstructing collaterals.Conclusion BMD changes in senile female patients with type 2 diabetes mellitus have no significant difference compared with those in healthy group in the same age,but decrease in male patients over 70.Osteoporosis is closely related to the syndromes of yin-deficiency,kidney-essence deficiency or blood stasis obstructing collaterals,which indicates that these syndromes are objective reality in the osteoporosis due to type 2 diabetes mellitus in senile patients.
