胶原涂层记忆合金人工气管的构建

摘    要:目的构建胶原涂层记忆合金人工气管,通过动物实验观察其气管重建的初步效果。方法以记忆合金丝编织成圆柱形网管作为基本支架,网管内壁涂聚氨酯膜封闭网孔,于管腔内外壁涂覆胶原海绵涂层构建胶原涂层记忆合金人工气管(A型人工气管)。同时构建无胶原海绵涂层的人工气管作为对照(B型人工气管)。15只杂种犬随机分为两组,分别以A型(n=8)和B型(n=7)人工气管行颈段气管的置换重建。通过X线摄片、气管镜和动物处死后组织标本的病理学检查评价实验效果。结果A型人工气管组6只动物存活超过90 d,分别于存活90、120、150 d后处死;术后人工气管在位佳,与自体组织融为一体;管腔通畅无狭窄,吻合口有气管上皮覆盖。B型人工气管组3只动物存活超过90 d,分别于存活90 d和150 d后处死;吻合口处无新生上皮覆盖,见不同程度的肉芽组织增生。结论胶原涂层记忆合金人工气管具有较好的生物相容性,能减少吻合口狭窄、裂开或假体脱位等并发症。

Abstract:Objective To investigate the feasibility of tracheal reconstruction with prosthesis made of memory alloy coated with collagen sponge in mongrel dogs. Methods The basic skeleton of prosthesis type A was knitted with Ni-Ti memory alloy filament.The tubular mesh was sealed with polyurethane membrane,and then inner and external wall of lumen was coated with collagen sponge.The constitution of type B prosthesis was similar with type A without coating collagen.Cervical trachea segmental reconstruction was performed in 15 mongrel dogs with type A(group A,n=8) and type B(group B,n=7) prosthesis.The efficacy of the implanted prosthesis was periodically evaluated using X-ray,tracheoscope and specimen microscope examination. Results In group A,six dogs survived over 90 d.They were sacrificed at 90,120 and 150 d after grafting.The implanted prosthesis was well located and completely incorporated with the recipient tissue,where re-epithelialization occulted on anastomotic sites.The tracheal lumen was patented. In group B,three dogs survived over 90 d.They were killed at 90 and 150 d after grafting.Various degrees of granuloma occurred and no re-epithelialization was observed in all dogs of group B. Conclusion Reconstructed tracheal prosthesis made of memory alloy coated with collagen sponge bears favourable biocompatibility and possess significant superiority to prevent anastomotic stenosis,decoherence and prosthetic dislocation.
