临终关怀对癌症病人家属生活质量的影响

目的研究临终关怀对癌症病人家属生活质量的影响。方法将2003年3月~2004年9月已病理确诊的60例临终癌症病人的家属作为研究对象,随机分为试验组和对照组。试验组33例,临终关怀的对象为病人和家属,除常规对病人进行护理以外,还对家属进行护理干预;对照组27例,临终关怀的对象为病人,只对病人进行常规的护理。结果除环境领域(P>0.05)外,试验组总的生活质量得分(P<0.001)、生理领域(P<0.001)、心理领域(P<0.005)、社会领域(P<0.05)得分与对照组比较有显著性差异。试验组与国内常模比较,无显著性差异(P>0.05);对照组与国内常摸比较有显著性差异(P<0.05)。各组病人家属在年龄、性别、婚姻状况、经济状况方面均有差异性(P<0.05)。2组病人家属总的生活质量评价和自身健康总的主观评价试验组优于对照组。结论临终关怀的对象不仅仅为病人,重要的是还要对病人家属进行心理支持,有利于家属顺利渡过居丧期,理智地复原,提高病人家属的生活质量,这对于提高社会人群的健康水平,减少疾病的发生起着重要的作用。因此,对临终病人家属的关怀不容忽视。

Influence of hospice care on quality of life of family members of cancer patients

Objective To study the influence of hospice care on quality of life of family members of cancer patients. Methods Family members of 60 hospice patients with terminal cancer were divided randomly into two groups: experimental group (33 participants received routine services to both patients and family members) and control group (27 participants received routine hospice services to patients only). The study was from March 2003 to September 2004. Results There were significant differences in scores of quality of life (P<0.001), physiological function (P<0.001), psychological function (P<0.005), social function (P<0.05) between two groups except for environmental functions (P>0.05). There were no statistically differences in these indexes between experimental group and internal norm (P>0.05). Significant differences in these indexes were observed between control group and internal norm (P<0.05). Quality of life and self-report health status of family members of the patients in experimental group were better than that of family members of the patients in control group.Conclusions Hospice services is not only suited for patients, but also for family members. It is in favor of family members to live through bereavement period smoothly, and to improve their life quality. It is propitious to boost the public health level and decrease diseases occurrence. So hospice care for family members of cancer patients should not be ignored.
