阿尔茨海默病患者DSC-MRI与认知功能的相关性

摘    要:目的 通过研究T*2加权动态磁敏感增强灌注成像(DSC-MRI)与阿尔茨海默病患者认知功能的相关性,评价DSC-MRI参数对AD患者病情评估中的作用.方法 选取2014年12月至2016年2月间收住于吉林大学中日联谊医院南湖院区神经内科符合临床诊断标准的30例AD患者,对所有病例采用1.5T磁共振扫描仪进行头颅平扫后再进行灌注扫描,随后将所得灌注图像传入后处理工作站,生成伪彩图及进行灌注参数测量,灌注参数包括脑血容量(CBV)、脑血流量(CBF)、平均通过时间(MTT)和达峰时间(TTP).分别对双侧额叶、颞叶、海马、海马旁回、杏仁体、楔前叶、基底节等区域进行测量,采用SPSS21.0统计软件分析,并将分析结果 与MoCA评分行相关性分析.结果在本次试验中MR灌注参数TTP与MoCA评分呈负相关,灌注参数CBV、CBF、MTT与MoCA评分无相关性.结论 脑DSC-MRI灌注参数TTP与临床认知功能结合对评估AD临床进展程度有一定帮助.

Abstract:Objective To evaluate the application value of DSC-MRI parameters in the assessment of AD's dis-ease through studying the correlation between T*2 weighted dynamic susceptibility enhanced perfusion imaging (DSC-MRI) and the cognitive function in patients with Alzheimer's disease (AD). Methods A total of 30 cases of AD patients, who meet the clinical diagnostic criteria and admitted to Department of Neurology of China-Japan Union Hospi-tal of Jilin University from December 2014 to February 2016, were selected as the research objects. 1.5T magnetic res-onance scanner was used for head scan and then perfusion scan in all the patients. The perfusion images were then transferred to the post-processing workstation to generate pseudo-color images and perfusion parameters. The perfu-sion parameters included cerebral blood volume (CBV), cerebral blood flow (CBF), mean transit time (MTT) and time to peak (TTP). On bilateral frontal lobe, temporal lobe, hippocampus, hippocampal side back, amygdala, precuneus, basal ganglia region were measured, and SPSS21.0 statistical software was used to analyze the correlation analysis with Montreal Cognitive Assessment (MoCA). Results This study showed that there was a negative correlation be-tween TTP and MoCA scores, and no correlation between CBV, CBF, MTT and MoCA scores. Conclusion Cerebral DSC-MRI perfusion parameters TTP combined with clinical cognitive function is helpful for estimating the clinical pro-gression of Alzheimer's disease.
