改良角膜隧道切口超声乳化白内障吸除术的临床研究

目的：探讨改良角膜隧道切口超声乳化白内障吸除术的临床疗效和安全性,以及对泪膜功能的影响。方法：选取符合标准的患者100例（108眼）,按随机数字表法分为观察组和对照组各50例,观察组行改良角膜隧道切口,对照组行透明角膜隧道切口。结果：术后两组患者裸眼视力较治疗前均显著改善（P〈0.05）,并呈进行性提高趋势;术后各时间点,观察组患者裸眼视力均优于对照组,差异存在统计学意义（P〈0.05）。两组患者术后各时间点角膜散光度与本组术前相比,差异存在统计学意义（P〈0.05）;术后各时间点,观察组患者角膜散光度均轻于对照组,差异存在显著性（P〈0.05）。术后3月,两组患者Sit值和BUT值接近术前水平,差异无统计学意义（P〉0.05）;术后1d、1周和1月,观察组患者Sit值和BUT值明显优于对照组,差异存在统计学意义（P〈0.05）。两组患者并发症发生率比较,差异无统计学意义（P〉0.05）。结论：改良角膜隧道切口超声乳化白内障吸除术具有临床疗效好,安全性高,短期内对泪膜功能影响小,值得临床推广应用。

Clinical Research on Phacoemulsification with Improved Cornea Tunnel Incisions

Objective： To investigate the clinical therapeutic effects and safety of phacoemulsification with improved cornea tunnel incisions,and the effects on lacrimal film function.Method： 100 patients（108 eyes）,meeting the criteria,were selected and randomly divided into observation group and control group,50 patients in each group;the observation group was treated with improved cornea tunnel incision,while the control group treated with transparent cornea tunnel incision.Result： After the surgery,the naked visions of the patients of the two groups were both significantly improved when compared with those before the surgeries（P0.05）;at each point after the surgeries,the naked visions of the patients of the observation group were all better than those of the control group,and the differences were statistically significant（P 0.05）.The differences in cornea astigmatism degree between before the surgery and each point after the surgeries of each group were statistically significant（P0.05）;at each point after the surgeries,the cornea astigmatism degrees of the patients of the observation group were all milder than those of the control group,and the differences were statistically significant（P0.05）.3 months after the surgery,the Sivt values and BUT values of both groups were close to those before the surgeries,and the differences were statistically insignificant（P0.05）;but 1 day,1 week and 1 month after the surgery,the Sivt values and BUT values of the observation group were significantly better than those of the control group,and the differences were statistically significant（P0.05）.The difference in the complication incidence between the two groups was statistically insignificant（P0.05）.Conclusion： Phacoemulsification with improved cornea tunnel incisions is clinically effective and highly safe,has few effect on lacrimal film function in short period,and is worthy of clinical application.
