肺间质性病变肺血管损害的比较影像学研究

摘    要:目的:通过肺间质性病变的动物模型,探讨在肺间质性病变中肺血管报害的不同影像学表现及其病理基础.材料和方法:应用肺间质性病变的动物模型,对其进行HRCT、MRI、DSA肺动脉造影检查和妥拉苏林试验,并和组织病理学对照。结果:HRCT表现有肺小叶间隔样结构增厚和支气管—血管束增粗及僵直、走行不规则等。MRI表现为肺血管走行、分布不自然,周围有信号异常。DSA肺动脉造影表现为肺血管纤细、缺失和呈残根状,妥拉苏林试验提示肺动脉对药物反应的顺应性降低。组织病理学表现肺血管管壁和鞘膜下结缔组织水肿、疏松、结缔组织增生和血管管腔狭窄和闭塞。结论:几种不同的影像学检查都能发现肺间质性病变所引起的肺血管的异常,但应用HRCT观察肺血管的损害、动态监测肺血管病变和肺内病变的发生、发展、和转归更为有效。

Abstract:Purpose: To study and observe the different imaging findings and pathologic based of pulmonary vessel' s le-sions in Pulmonary interstitial disease through model of animal of pulmonary interstitial disease. Materials and methods: Using the models of animal of pulmonary interstitial diseases examined by HRCT,MRI,pulmonary arterial angiography DSA and To lazoline test. The imaging findings compared with pathology. Results: The interlobular septa-like structure and bronchovascular bundles were thichened, rigid and unregular on distribution on HRCT. The pulmonary vessel's distribution was abnormal and there was abnormal signal around pulmonary vessels on MRI. The Pulmonary vessels were thin, omitted and incomplete on pulmonary arte-rial angiography DSA. The pulmonary arterial elastiCity was loss on Tolazoline test. The wall and sub - tunica uaginalis of pul-monary arterioles were found with congestion, edema, cellular infiltration, slightly hyperplasia of fibrous tissue, vessels narrow and obliteration on pathologic examination. Conclusion: The pulmonary vascular lesions can be found on different imageology ex-amination, but HRCT is the best method to observe pulmonary vessel's lesion and pulmonary lesion in interstitial lung disease.
