肝细胞癌MR信号强度与组织学和DNA含量的关系

摘    要:用0.35T磁共振仪检查41例肝细胞癌(HCC),均经手术、病理证实,将磁共振信号强度按T1/T2分为稍低/稍高信号、低/高信号及信号不均3组;组织学上分为梁索优势型、假腺管优势型及实质优势型;用图像分析仪测量每例癌细胞核DNA含量,计算DNA指数(DI)及倍型。分析肝细胞癌MR信号与组织学和DNA含量的相关性。结果显示,稍低/稍高信号组,以梁索优势型为多,占82.6%,平均DI为2.39;低/高信号组,以实质优势型为多,占71.4%,平均DI为3.78,二者差异显著,结果表明,HCC的MR信号改变在一定程度上反映了不同的组织学类型和DNA含量的高低。

Abstract:Forty one cases of hepatocellular carcinoma(HCC) were exarnined by a 0.35T MR system,all were proved by surgery and histology Signal intensity of MR were classified into 3 groups,i.e. slightly hypointense on T1 WI and slightly hyper intense on T2 WI; slightly hypointense on T1 WI and very hyper intense on T2 WI; heterogeneity signal on both T1 and T2 WI.The histologic patterns were classified into trabecular pattern, pseudoglandular pattern and solid pattern The nuclear DNA content was determined by color imaging analysis technology and the DNA index(DI) and ploidy were calculated .In this study we correlated the signal of MR with histology and nuclear DNA content of HCC. The results showed most of the cases in slightly hypointense/slightly hyperintense group were of trabecular pattern(19/23),in which the mean DI was 2.39.In hypointense/hyperintense group the histologic changes were of solid pattern(5/7),with the mean DI of 3.78.There were statistical difference of DI between the two groups(P< 0.01)Conclusion: MR signal intensity of HCC reflects histologic type of the tumor and nuclear DNA content in a certain degree.
