尿毒症的可逆因素及其临床意义

目的　探讨有可逆因素的可逆性尿毒症的治疗价值 ,以及可逆因素的常见顺序 ,为尿毒症的内科保守治疗提供参考依据。方法　比较有明显可逆因素及无明显可逆因素的两组尿毒症患者 ,在经过单纯内科保守治疗后 ,临床表现及肾功能改善程度的差异 ,并统计可逆性尿毒症可逆因素的出现率。结果　可逆性尿毒症组在经过包括纠正可逆因素的保守治疗后 ,临床症状及肾功能改善明显优于无可逆因素的尿毒症组。可逆因素以感染和脱水最为常见。结论　纠正可逆因素是改善临床症状及肾功能的关键 ,寻找可逆因素尤应注意呼吸系统和泌尿系统感染及医源性脱水。

Reversible factors of uremia and its clinical significance

Objective To re search the therapeutic potentials of reversible uremia patients with reversible factors and occurrence sequence of the reversible factors to provide scientific basis for the internal conservative treatment of uremia.Methods The differences of clinical manifestations and the improvemen of renal function were compared in two group of uremic patients with or without obviously reversible factors undergone internal conservative treatment, and make a occurrence statistics of reversible factors of reversible uremic patients.Results After conservative treatments in the reversible uremic patients included in corrected reversible factors, their clinical manifestations and renal function improvement were significantly better than that of uremic patients without reversible factors. The most common reversible factors were infection and dehydration.Conclusion To correct the reversible factors is the key step to improve the clinical manifestations and renal function The findings of reversible factors should be taken care of infection and dehydration. Especially, the infection of respiratory and urinary systems and iatrogenic dehydration should be paid more attention.
