臀部损伤致大出血27例分析和处理

目的探讨臀部损伤合并血管损伤致大出血救治的程序、止血的路径和方法.方法总结分析27例臀部外伤所致臀部大出血的治疗经过.结果15例局部伤口扩大止血成功;4例腹膜外结扎同侧髂内动脉止血成功;5例剖腹结扎同侧髂内动脉止血成功;2例结扎双侧髂内动脉止血成功;1例结扎双侧髂内动脉仍出血不止,扩大局部创口结扎出血血管止血成功.结论臀部的深部创伤,伴有大出血者,都应考虑到臀部大血管损伤可能,同时要警惕腹腔脏器损伤的可能,其处理要及时和方法要得当,以降低死亡率.

Analysis and disposal of hematorrhea in 27 cases of haunch injury

Objective To investigate the procedure of disposal of massive hemorrhage cuased by haunch injury complicated with blood vessel injury and the routes and methods of hemostasis.Methods To summarize and analyze the treatment course of hematorrhea caused by haunch injury in 27 patients.Results Fifteen patients were stanched successfully by extending local wound;4 patients by ligation of internal iliac artery through extraperitoneum;5 patients the ipsilateral by ligation of internal iliac artery through laparotomy;2 patients the ipsilateral by ligation of bilateral internal iliac arteries;1 patient still bled through the both sides of internal iliac artery ligation,then was stanched successfully by hemorrhagic blood vessel ligation through extending local wound.Conclusion Deep injury of haunch accompanied with hematorrhea was likely complicated with big blood vessel injury of haunch, meanwhile, we should be alert to intra-abdominal organ injury,then treat them properly in time to decrease its mortality.
