肝硬化伴肝肺综合征影像学表现分析

目的：讨论肝硬化伴肝肺综合征的影像学表现，分析其中意义。方法：对2004—2010年临床诊断的有完整资料的21例肝硬化伴肝肺综合征的胸部影像学表现进行回顾性分析。胸部X线检查21例，CT检查7例。结果：胸部异常表现19例，X线主要表现为以中下肺为主的斑点状、粟粒状阴影或呈“海绵状”间质性改变。CT主要表现为双下肺小结节状、网状或蜂窝状阴影，以上表现具有一定特异性。结论：除临床症状与血气检测等外，正确认识胸部影像学表现，对肝硬化伴肝肺综合征的诊断、治疗及判断预后具有重要意义。

Imaging Analysis of Hepatocirrhosis with Hepatopulmonary Syndrome

Objective： To analysis the imaging of hepatoeirrhosis with hepatopulmonary syndrome and find its clinical signifi cance. Methods： 21 cases of hepatoeirrhosis with hepatopulmonary syndrome proved by clinic during 2004 to 2010 were retrospectively analyzed the imaging of ehest, including 21 radiograms and 7 CT scans. Results： 19 eases were found abnormal appearance, the chest radiograms feature were mottling, reticulate shadows and sponginess interstitial infiltration in the middle and lower lung, CT scans feature were lesser nodular, reticular, honeycomb shadows in the lower lung. Conclusion： In addition to clinical symptoms and blood gas analysis, the chest imaging is very in4portant for the diagnosis, therapeutics and prognosis of the hepatoeirrhosis with hepatopulmonary syndrome.
