原发性肝细胞癌超声造影峰值时间与肿瘤动脉密度相关性研究

摘    要:目的探讨原发性肝细胞癌超声造影峰值时间与肝癌动脉密度之间的相关性及临床意义。方法分析18例应用SonoVue进行超声造影的原发性肝细胞癌患者的声像图,确定肝癌结节的造影增强峰值时间,然后对该肿瘤结节术后病理切片在光学显微镜下进行观察,计算肿瘤内动脉血管密度(根/高倍视野),对两项观察结果进行统计学分析。结果18例患者共观察18个肿瘤结节,造影剂平均峰值时间为(18.9±2.8)s,肿瘤血管密度平均为(0.17±0.06)根/高倍视野,本组资料显示原发性肝癌造影峰值时间与肝癌瘤内动脉血管密度之间存在负相关关系,其相关系数r=-0.72,且有统计学意义(P<0.05)。结论本组资料显示原发性肝细胞癌超声造影的峰值时间与瘤内动脉密度呈明显的负相关关系,这提示通过造影峰值时间可估计肿瘤结节内的血供状况,由此可了解原发性肝细胞癌的某些生物学特性,有一定的临床意义。

Abstract:Objective To approach the relation of primary hepatocellular carcinoma (PHCC) between peak time of contrast enhanced ultrasound (CEUS) and artery density and its clinical value. Methods Eighteen cases with PHCC were examined by CEUS and the peak time was recorded. After operation, the pathological sections of all cases were observed with light microscope and the artery density was calculated. Then the correlation between the peak time and the artery density was analyzed. Results In all nodules, the average peak time of CEUS was (24.60±4.33) s and the average artery density of PHCC was 0.17±0.06/HP. The correlation coefficient between them was -0.72 and it was statistically significant (P<0.05). This indicates the peak time of CEUS become shorter as the artery density become higher. Conclusion The peak time of CEUS is inversely correlated with artery density of PHCC. This indicates we can estimate the blood supply by peak time of CEUS and the biological character for clinical therapy.
