会厌及双蒂甲状软骨膜带状肌瓣一期喉功能重建

目的观察会厌及双蒂甲状软骨膜带状肌瓣一期修复残喉的价值。方法从1992-03～2001-07 对12例声门癌、5例声门上癌接受喉部分切除术,其残喉用会厌及双蒂甲状软骨膜带状肌瓣进行一期修复。结果 11／12声门癌顺利地拔除了气管套管并恢复了喉的功能,5例声门上癌全部恢复了喉的功能, 并于术后15～20d恢复了吞咽功能,3年生存率为76.5％。结论会厌及双蒂甲状软骨膜带状肌瓣一期修复残喉具有取材容易,修复方法简单,且明显地提高了患者术后的生存质量。

The application of bi-pedical strap muscular flap with throid perichondrium and epiglottis in the reconstruction of larynx function after partial laryngectomy

Objective To observe the applicable value of bi-pedical strap muscular flap with throid perichondrium and epiglottis in the reconstruction of larynx function after partial.Method From March, 1992 to July, 2001, the 17 cases of glottic and superglottic carcinomas of larynx had received partial laryngectomy and the residual larynges were repaired using bi-pedical strap muscular flap with throid perichondrium and epiglottis.Results Eleven of the twelve glottic carcinomas and five supraglottic carcinomas patients were decannulated and recovered laryngeal functions,and the patients rehabilitated deglutition later 15-20 days after surgery. Three-year survival rate was 76.5%.Conclusion Epiglottis and bi-pedical strap muscular flap with throid perichondrium have many advantages in the reconstruction of larynx function after partial laryngectomy.lt is easy to approach and apply, and can improve the life quality of the patients underwent partial laryngectomy.
