胸腔镜下滑石粉胸膜固定术治疗恶性胸腔积液疗效分析

目的观察胸腔镜辅助下医用滑石粉胸膜固定术治疗大量恶性胸腔积液的疗效，探讨其应用价值。方法将2006年1月至2008年12月本科收治的74例恶性胸腔积液患者分成两组，采用胸腔镜下医用滑石粉胸膜固定术35例，采用胸腔闭式引流（ARROW管）并注入50％高渗葡萄糖治疗39例。结果胸腔镜辅助下医用滑石粉胸膜固定术组总有效率88．6％，其中82．9％的恶性胸腔积液患者获完全持久胸膜固定；高渗葡萄糖胸膜粘连术治疗恶性胸腔积液组总有效率51．2％，其中完全缓解38．5％。结论胸腔镜辅助下医用滑石粉胸膜固定治疗恶性胸腔积液是一种有效措施。

Clinical analysis on the therapeutic effects of treating malignant pleural effusion with Talc Powder pleurodesis under thoracoscope

Objective To observe the therapeutic effect of treating malignant pleural effusion with talc powder pleurodesis thoracoscopically and to explore its clinical value. Methods 74 patients of malignant pleural effusion from Jan 2006 to Feb 2008 were divided into two groups, thoracoscopic talc powder pleurodesis was applied in 35 cases, closed drainage of thoracic cavity was applied（ with ARROW tube） and hypertonic glucose was injected in 39 cases. Results The total effective rate in thoracoscopic talc powder pleurodesis was 88.6% , among them, 82.9% patients had a completely permnant pleure fixation, in the group of hypertonie glucose, the total effective rate was 51.2 % , with 38.5 % of complete remission. Conclusions Treating malignant pleural effusion with talc powder pleurodesis thoracoscopically is an effective method.
