重症急性胰腺炎大鼠心肌核因子-κB激活和益活清胰汤的干预作用

摘    要:目的:研究重症急性胰腺炎(SAP)心肌功能损害时心肌NF-κB的活化及益活清胰汤的干预作用。方法:63只SD大鼠随机分为假手术组(n=9)、对照组(n=27)、益活清胰汤治疗组(n=27),SAP模型采用5%的牛磺胆酸钠胰胆管逆行注射方法建立。ELISA法测定6 h、12 h、24 h各时点血TNF-α、IL-6含量,RT-PCR检测心肌NF-κB mRNA的表达,免疫组化法检测心肌NF-κB蛋白的表达,HE染色光镜下观察胰腺及心肌组织的病理变化。结果:术后6 h大鼠心肌NF-κBmRNA及NF-κB蛋白表达异常增高,TNF-α、IL-6呈进行性升高,治疗组用药后心肌NF-κBmRNA及蛋白表达下调,与对照组相比(P0.05),血TNF-α、IL-6明显下降,与对照组相比(P0.05),治疗组胰腺及心肌组织的病理变化减轻。结论:SAP大鼠的心肌损伤可能与循环中TNF-α、IL-6水平升高导致的心肌NF-κB活化有关,益活清胰汤对SAP并发的心肌损伤具有防治作用。

Myocardial NF-κB Activity in SAP Rats and Intervention Effect of Promote Blood Circulation Qing-yi Decoction

Abstract:Objective: To investigate myocardial NF-KB mRNA activity in the pathogenesis of severe acute pancreatitis-associ- ated myocardial damage and the intervention effect of the Promote Blood Cireulation Qing-yi Decoction on it. Methods: Sixty-three male SD rats were divided randomly into the sham-operated group ( A group, n = 9) , the control group ( B group, n = 27) and the Promote Blood Circulation Qing-yi Decoction plus recombinant staphylokinase (r-SAK) treatment group (r-SAK) ( C group, n = 27 ) . recom- binant staphylokinase treatment group (C group, n = 27) . SAP was induced by intraductal administration 5% sodium taurocholate. Artery blood of the A, B, C group was obtained at 6h, 12h and 24h after induction of the model respectively. Serum concentrations of TNF- α and IL-6 were measured with ELISA. The mRNA expression of NF-KB in myocardium tissue was evaluated by RT-PCR. The myocardium tissue of NF-κB protein was determined by SP immunohistochemical technique . Pancreatic and myocardial tissues were stained with HE and examined under light microscopy. Results: The myocardial NF-KB expression and the serum levels of TNF- α and IL-6 was significantly increased in 6 hours after induction of SAP. Myocardial NF-KB expression of the treatment group and serum TNF- α, IL-6 were significantly decreased , comparing with those of the control group ( P 〈 0. 05) . The treatment group of pancreatic andmyocardial tissues in pathological changes was alleviated in contrast to the control group. Conclusion: The myocardial damage might attribute to myocardial NF-κB activation due to an increase inTNF- α , IL-6 of the blood circulation in SAP rats. The Promote Blood Circulation Qing-yi Decoction have an beneficial role in the prevention and therapy of SAP associated myocardial damage.
