慢性心力衰竭患者睡眠质量及其影响因素调查分析

目的了解慢性心力衰竭(心衰)患者住院期间的睡眠质量及其影响因素.方法采用匹兹堡睡眠质量指数(PSQI)量表,焦虑自评量表(SAS),抑郁自评量表(SDS)和睡眠影响因素调查表对100例慢性心衰患者进行问卷调查.结果 73.0%慢性心衰患者有睡眠问题,PSQI总分及7个分项得分与国内常模相比较,差异有显著性意义(均P<0.01);心功能等级、SAS、SDS和环境质量与睡眠质量呈正相关关系;影响因素依次为生理(呼吸困难、心悸、咳嗽、咳痰),心理(担心疾病、经济困难、家庭功能不良)和环境(院内噪声、夜间治疗护理)三方面.结论慢性心衰患者的睡眠质量较差,应采取有效的护理措施控制影响睡眠的各种因素.

Investigation of Sleep Quality and Influencing Factors in Patients with Chronic Heart Failure

Objective: To study sleep quality and the influencing factors in chronic heart failure patients during their hospitalization.Methods A total of 100 patients with chronic heart failure were assessed by using the Pittsburgh Sleep Quality Index (PSQI), the Zung Depression (SDS) and Anxiety Scales (SAS) and sleep affecting factors questionnaire.Results Among the 100 patients, 73.0% patients were troubled by sleep problem, whose scores of PSQI global sum and 7 components were quite different from Chinese Norm (P<0.01). The stage of heart failure, SAS, SDS and environment quality was positively correlated with sleep quality significantly. The influencing factors were in order physiological (dyspnea, cardiopalmus, cough, expectoration), psychological (worries about disease, economic hardship, family dysfunction), and environmental factors (noises in hospital, therapeutic nursing at night).Conclusion Chronic heart failure patients had a relatively poor sleep quality, and effective nursing measures should be taken in order to ameliorate their sleep quality by effectively controlling the factors which affect sleep.
