肺癌病人最大呼吸流速容量曲线及弥散功能变化的意义

摘    要:目的了解肺癌患者肺功能最大呼吸流速容量曲线(MEFV)变化的意义及其变化与气道阻力(Raw)、用力肺活量(FVC)测定相关性。观察放/化疗后弥散功能和肺CT的改变。方法测定36例肺癌患者的MEFV、Raw及弥散功能,并对其间的相关性进行分析。结果发现肺癌病人存在FVC、FEV1、PEF、V!50、V"25下降,且这种下降在放/化疗后更明显,PEF、V#50下降与吸/呼气的气道阻力呈负相关,与FVC呈正相关。肺癌患者在放/化疗后1、2年Dlco、Co减少,CT所见存在28.57%肺间质病变。结论肺癌病人肺功能表现可有阻塞性通气功能障碍,当气道阻塞严重时,PEF、V$50、V%可同时下降,说明PEF的下降可与小气道的阻塞有关,V50下降也可与用力大小有关。DLco可与CT一样作为敏感指标观察放/化疗后肺间质的损害。

Abstract:Objective To explore pulmonary function maximal expiratory flow volume curve(MEFV)and relation between MEFV and airway resistance(Raw),forced vital capacity(FVC)with lung cancer.To study the changes of diffuse function(DLCO)and CT scan after chemiotherapy and roentgenotherapy.Methods MEFV,RAW,DLCO of 36 patients with lung cancer were measured.Results Forced expiratory volume in one second(FEV1),peak expiratory flow(PEF),maximal expiratory flow in 50% vital capacity(V50)of patients with lung cancer were decreaed in comparison with healthy controls.There was clearly that pulmonary function index was decreaed after chemiotherapy and roentgenotherapy.There was impositive relation between PEF,V50 and RawInsp,RawExp.There was positive relation between PEF,V50 and FVC.DLco,CO of patients with lung cancer were decreaed in 1 or 2 years using chemiotherapy and roentgenotherapy.Meanwhile,there was 28.57% with diffuse parenchymal lung disease by CT scan.Conclusions There was obstructive ventilatory defect in lung cancer patients.When airway obstruction was severe,PEF,V50,V25 were all lower on time.These changes was relating to small airway obstruction and muscular force.DLco was sensitive index to see diffuse parenchymal lung disease after chemiotherapy and roentgenotherapy.
