中西医结合治疗早中期膝骨性关节炎疗效观察

目的：观察玻璃酸钠＋曲安奈德关节腔注射,配合自拟中药内服及熏蒸加功能锻炼治疗膝关节早、中期骨性关节炎的临床疗效.方法：共收治58例膝关节早、中期骨性关节炎患者,在行玻璃酸钠＋曲安奈德关节腔内注射的同时,予以自拟通痹消肿颗粒内服,配合滑膜炎洗剂熏蒸,另外积极进行膝关节功能训练,进行综合治疗.结果：所有患者经综合治疗后,临床症状都得到不同程度的改善,按日本骨科学会膝关节JOA评分标准中,评分明显改善,按《中药新药治疗骨性关节炎的临床研究指导原则》进行评估,总有效率96.5%.结论：自拟中药内服、外洗配合玻璃酸钠＋曲安奈德关节腔注射,外加膝关节功能训练等综合疗法治疗早、中期膝关节骨性关节炎,临床疗效满意,可有效缓解疼痛,改善关节功能,且有效延缓了膝关节退变进程的加剧和关节畸形的发生,从而避免了膝关节置换手术干预的年轻化。

Observation of Curative Efficacy of Integrated Treatment of TraditionalChinese Medicine and Western Medicine on Knee Osteoarthritis at Early andMedial Stages

Objective： To observe the clinical curative efficacy of integrated treatment of Traditional Chinese Medicine and westernmedicine on treating knee osteoarthritis at early and medial stages. The treatment included intra-articular injection of sodium hyaluronateand triamcinolone acetonid, oral administration of Chinese medicine, Chinese herbal fumigation and functional exercise. Methods： 58patients with knee osteoarthritis were given intra-articular injection of sodium hyaluronate and triamcinolone acetonid. At the same time,the self-made Tongbi Xiaozhong granules were orally taken by the patients. The patients also took fumigation with lotion for synovitis andfunctional exercise. Results： After the comprehensive treatment, the clinical symptoms of all patients were improved in different level. Forthe Japanese orthopaedics association score, all scores of the patients were significantly improved. In the evaluation according to theGuiding Principles for Clinical Research on Treatment of Knee Osteoarthritis with New Chinese Medicine, the total effective rate was96.5%. Conclusion： The comprehensive treatment including oral administration of self-made Chinese medicine, fumigation, intra-articularinjection of sodium hyaluronate and triamcinolone acetonid and functional exercise of knee for knee osteoarthritis at early and medialstages has satisfying clinical effect. It can relieve pain, improve joint function, delay the degeneration and malformation of knee joint, sothe younger trend of knee replacement can be avoided.
