冲洗引流对食管贲门癌术后胸内吻合口瘘患者预后的影响

目的 观察氯霉素液冲洗引流对食管癌和贲门癌切除术后胸内吻合口瘘患者预后的影响.方法 将38例胸内吻合口瘘患者随机分为两组,其中治疗组20例,用生理盐水加入庆大霉素冲洗瘘口,口服氯霉素液从胸腔引流管冲洗胸腔;对照组18例,每日仅用生理盐水加入庆大霉素从胸腔引流管冲洗胸腔.观察两组患者血象恢复正常时间、瘘口愈合时间及死亡率情况.结果 与对照组相比,治疗组血象恢复正常时间、瘘口愈合时间明显缩短,死亡率明显降低,两组比较具有统计学差异（P〈0.05）.结论 用生理盐水加入庆大霉素冲洗配合口服氯霉素液冲洗瘘口,可减少感染程度,促进瘘口愈合,降低死亡率,疗效优于常规治疗.

The Effect of Alficetin Wash and Drainage Schema on the Patients of Thoracic Cavity Stoma Fistula of Esophageal Carcinoma and Carcinoma of Gastric Cardia

Objective To observe the therapeutic effect of alficetin wash on thoracic cavity stoma listula of esophageal carcinoma and carcinoma of gastric cardia. Methods 38 cases patients with thoracic cavity stoma fistula were devided into two groups, the treatment group （n=20） were treated by washing orificium fistulae with isotonic Na chloride addition of cidomycin and alficetin for oral use： the control group. （n=18） were treated by washing orificium fistulae with isotonic Na chloride addition of cidomycin. The time of orificium fistulae concrescence and prognosis condition were observed.Results Compared with control group, the time of orificium fistulae concrescence were shortened, death rate were decreased in the treatment group, which was different from that of control group （P〈0,05） .Conclusion washing orificium fistulae with the isotonic Na chloride addition of cidomycin and alficetin for oral use could sborten the revive time of body temperature and the time of orificium fistulae concrescence, decrease death rate, and the curative effect were better the conventional therapy.
