应用ROC曲线分析法评价免疫重氮乳凝试验诊断结核病的价值

目的　评价新型免疫载体重氮乳胶凝集试验 (DLAT)在结核病临床诊断中的价值 ,为临床应用提供参考依据。方法　选择临床确诊的结核病人和无结核病史的正常人为研究对象 ,收集相应的临床检测标本。以BACTEC快速培养和ELISA两种实验室常用的结核诊断方法为对照。根据每种方法对标本检测结果的灵敏度和特异度 ,以受试者工作特性曲线(ROC)进行分析比较。结果　DLAT ,BACTEC培养和ELISA三种方法的灵敏度分别为 6 1 32 % ,5 3 77%和 6 2 2 6 % ;假阳性率分别为 2 30 % ,0和 4 6 0 % ;ROC曲线下的面积分别为 0 795 ,0 76 9和 0 788。P值均小于 0 0 1。结论　ROC曲线分析结果显示 ,重氮乳凝法诊断结核病具有较好的准确性 ,该方法可作为结核病的实验室诊断方法

Evaluation of diagnostic accuracy of diazopolystyrene latex agglutination test in identifying pulmonary tuberculosis using ROC curve analysis

Aim To evaluate the diagnostic accuracy of diazopolystyrene latex agglutination test(DLAT)in identifying pulmonary tuberculosis patients,thus to provide reference basis for clinical diagnosis Methods 106 patients with pulmonary tuberculosis and 87 health people were observed,and corresponding specimen were collected and detected by BACTEC quick culture method and ELISA in laboratory The sensitivity and specificity of detected results were analyzed by receiver operating characteristic(ROC)curve Results The sensitivity of the DLAT,ELISA and BACTEC were 61 32%,62 26% and 53 77% respectively Their false positive rate were 2 30%,4 60% and 0 respectively And the area under ROC curve were 0 795,0 788 and 0 769 respectively Conclusion DLAT is provided with valuable detecting index for diagnosing TB cases using ROC curve analysis This method may be applied to clinical diagnosis
