术前营养不良对克罗恩病病人术后并发症和复发的影响

摘    要:目的:讨论克罗恩病(CD)病人术前营养状况对术后并发症和复发的影响。方法:回顾性分析94例因CD相关并发症行肠切除吻合手术病人的临床资料,比较营养正常组(41例)与营养不良组(53例)病人术后内镜复发、临床复发、并发症发生率、拆线时间、术后住院天数和住院费用等情况。结果:两组病人术后并发症发生率、拆线时间、术后住院天数比较差异均有统计学意义(P均<0.01)。分别比较两组术后1、2和3年的累计内镜复发和临床复发率,差异均无统计学意义。结论:CD病人术前营养状况良好可显著减少术后并发症的发生率,加速切口愈合,缩短住院天数,减少费用。术前营养状况可能对术后复发无明显影响。

Abstract:Objective: The present retrospective study was aimed to determine whether perioperative malnutrition was associated with postoperative recurrence and complications after bowel resection for Crohn’s disease.Methods: Patients undergoing bowel resection for CD at the Jinling hospital of Nanjing University School of Medicine from 2002 to 2011 were retrospectively analyzed.Postoperative recurrence and complications in patients without malnutrition were compared with those with undernutrition.Results: Of the 94 patients,53 were malnourished.Clinical and endoscopic recurrence were recorded and compared respectively.The difference about postoperative recurrence between the two groups was insignificant.But the rates of postoperative complications in patients without malnutrition were significantly lower than those with malnutrition(P<0.01).Conclusion: Although nutritional status showed no significant effects on postoperative recurrence,patients without malnutrition at the time of surgery have decreased complications after operation.
