心肌缺血预适应对心肌的梗死面积及梗死后左室功能的影响

目的：探讨心肌缺血预适应对梗死心肌的保护作用。方法：对２５例急性心肌梗死（ＡＭＩ）患者按发病前２４ｈ内有无心绞痛分为有缺血预适应组（ＩＰ，１２例）及无缺血预适应组（ＮＩＰ，１３例），并对各组患者的左室舒张功能和心肌梗死面积进行统计分析。结果：ＩＰ的心肌酶ＣＫ，ＣＫ－ＭＢ峰值、心电图估测的梗死面积及左室舒张功能指标二尖瓣血流Ａ／Ｅ比值均低于ＮＩＰ组，而左室收缩功能指标左室短轴缩短率（ＦＳ）无明显变化

EFFECT OF ISCHEMIC PRECONDITIONING IN INFARCT SIZE OF MYOCARDIUM AND FUNCTION OF LEFT VENTRICLE AFTER ACUTE MYOCARDIAL INFARCTION

Objective: Assess the protective effects of ischemic preconditioning to myocardium in acute infarction;Method: According to the state of angina pectoris within 24 hours before acute myocardial infarction(AMI), twenty five patients were classified into two groups,12 patients with ischemic preconditioning (IP) and 13 without ischemic preconditioning (NIP).Then measured and investigated the criteria of the diastolic function and infarct size of left ventricle. Result: The peak value of myocardial enzyme CK and CK-MB, and the estimated infarct size of myocardium by ECG ,also the criteria of diastolic function A/E ratio in IP group proved to be lower than that in NIP group. However,the criteria of systolic function fractional shortening (FS) was not significantly different between two groups. Conclusion: The ischemic preconditioning has effects on protection of myocardium, decrease of infarct size and improvement of the diastolic function in left ventricle.
