血清乙型肝炎病毒前S1抗原检测在预防性健康体检中的应用

目的]探讨血清前Sl抗原检测在预防性健康体检中的应用价值。方法]用双抗体夹心ELISA法检测233例健康体检者乙型肝炎病毒(HBV)及正常人血清中HBV前S1抗原(Pre-S1),并将其结果与荧光定量HBVDNA、HBVM及ALT的检测结果进行比较。结果]在各组HBV感染者中均检出Pre-S1,检出率为20%~75%。HBeAg阳性组中检出率和相对滴度显著高于HBeAg阴性组(P<0.001);Pre-Sl和HBVDNA检测结果符合率达81.12%,Pre-Sl的相对滴度与HBVDNA含量之间呈正相关(r=0.9293,P<0.001);HBeAg阴性组ALT异常血清的Pre-Sl阳性率高于ALT正常血清(P<0.05),同是ALT异常,则HBeAg阳性血清的Per-SI阳性率高于HBeAg阴性血清(P<0.05)。结论]血清前Sl抗原与HBV活动性复制、肝细胞损伤关系密切,可作为健康体检乙型肝炎传染性筛选的重要补充。

Application of detecting serum hepatitis B virus pre - S1 antigen in physical examination

Objective] To evaluate the value of detecting serum hepatitis B Virus(HBV)Pre-Sl antigen in the physical examination. Methods] Pre-Sl antigen of 233 serum samples were tested by enzyme-linked immuno-sorbent assays(ELISA),and the results were compared with those obtained from HBVM,HBVDNA and ALT. Results] The positive rate of pre-S1 was 20% to 75% in different groups of people infected with HBV.The difference in positive rate and relative titer between HBeAg positive and negative groups were significant(P<0.001).The titer of pre-Sl was positively related to HBVDNA concentration,with correlation coefficient 0.9293(P<0.001).Among abnormal cases of serum ALT in HBeAg negative group,the positive rate of PreSl was significantly higher than among normal cases of serum ALT(P<0.05).Among the abnormal cases of serum ALT,the positive rate of Pre-Sl in HBeAg positive group was significantly higher than in HBeAg negative group (P<0.05).Conclusion] The results showed that Pre-Sl is closely associated with active replication of HBV and liver cells injury,and that is an important serologic marker for monitoring HBV infection.This can be an important supplement in physical examination for screening the infectivity of hepatitis B.
