早期气管插管对急性重度有机磷中毒并呼吸衰竭的临床观察

目的探讨气管插管时机对急性重度有机磷中毒(ASOPP)治疗效果的影响。方法分析68例急性重度有机磷中毒伴呼吸衰竭患者资料,其中分治疗组38例,为早期气管插管组,对照组30例,为延迟气管插管组,比较两组患者的抢救效果。结果早期气管插管组与延迟气管插管组比较,机械通气时间、住ICU时间、住院时间均显著缩短(P<0.01),呼吸机相关性肺炎发生率显著降低(P<0.01),治愈率显著提高。结论早期气管插管救治重度有机磷农药中毒患者疗效确切,气管插管最佳时机为呼吸衰竭早期。

Effects of early tracheal intubation on acute serious organophosphate poisoning combined with respiratory failure

Objective To evaluate the value of early tracheal intubation on acute serious organophosphate poisoning(ASOPP) patients.Methods Clinical data of 68 ASOPP patients with respiratory failure(RF) were retrospectively analyzed.The patients were divided into early tracheal intubation group(n=38) and delay tracheal intubation group(n=30),and the therapeutic effect of two groups was compared.Results Time of mechanical ventilation,days of ICU stay and length of stay significantly decreased,incidence of ventilator-associated pneumonia significantly decreased,and cure rate significantly increased in early tracheal intubation group(P<0.01) compared with delay tracheal intubation group.Conclusion It is certain that the therapeutic effect of early tracheal intubation on rescuing the ASOPP patients,and the best time to tracheal intubation is in early period of RF.
