摘    要:目的 探讨液体管理对重症肺炎机械通气影响.方法 157例重症肺炎分为液体管理组和对照组,脉搏指示连续心输出量监测(PiCCO)指导液体管理,测定肺组织局部IL-6、胸部SCT检查及血浆脑型利钠肽(BNP)了解心肺情况,统计机械通气时间及参数变化.结果 两组相比,液体管理组机械通气时间缩短(P<0.05 ),呼吸机支持力度下降(P<0.05 ),IL-6、BNP均降低(P<0.05),胸部CT病变好转,心肺功能改善.结论 强化液体管理可早脱机、减小呼吸机支持,与改善心肺功能有关.

Abstract:Objective To explore the impact of strict fluid management on mechanical ventilation in severe pneumonia and its mechanisms.Methods 157 cases of patients with severe pneumonia were randomly divided into fluid management and control groups.The pulse indicator continuously monitored the condition of cardiac output to guided the liquid adjustment,lung tissue IL-6,chest CT scan and plasma brain natriuretic peptide(natriuretic peptide,BNP) were tested for heart and lung function,mechanical ventilation time and parameter variations were recorded.Results Compared two groups,fluid management group shorten the duration of mechanical ventilation(P<0.05),ventilator support decreased(P<0.05),IL-6,BNP decreased(P<0.05),chest CT lung lesions extenuated,and cardio pulmonary functional improved.Conclusions The strict fluid management can reduce ventilator support,and improve heart and lung function.
