胸腔内注射不同药物治疗肺癌胸腔积液疗效对比观察

目的观察不同药物治疗恶性胸腔积液的疗效。方法肺癌恶性胸腔积液66人,随机分成四组:顺铂组50 mg/m2;第1～5天,注药前后充分止呕吐,3周后重复一次;IL-2组200 IU第1～7天,3周后重复一次;金葡素组200 ng(20 ml)1次/周,连用4周;顺铂+金葡素组:顺铂50 mg/m2+金葡素200 ng(20 ml)1次/周,连用4周。结果顺铂组疗效36.8%,IL-2组疗效41.6%,金葡素组疗效81.25%,顺铂+金葡素组疗效89.4%。金葡素组疗效高于顺铂组,有显著差异(P<0.01),也高于IL-2组(P<0.01)。顺铂+金葡素组疗效高于顺铂组,有显著差异(P<0.01),也高于IL-2组(P<0.01)。结论顺铂、IL-2、金葡素及顺铂+金葡素治疗肺癌胸腔积液均有效。顺铂+金葡素疗效并不高于单药金葡素。

Curative effect of intrathoracic injection of various medicines on pleural effusion caused by lung cancer

Objective To study the curative effect of intrathoracic injection of various medicines on pleural effusion caused by lung cancer.Methods 66 patients of pleural effusion with lung cancer were randomly divided into four groups: cisplatin(groupⅠ),IL-2(group Ⅱ),Staphylococcin(group Ⅲ) and Cisplatin plus staphylococcin,(group Ⅳ).Cisplatin was injected into thoracic cavity by 50 mg/m2 from first day to fifth day,and repeated after three weeks;Staphylococcin was injected by 200 ng,1 times a week for 4 consecutive weeks.Cisplatin plus staphylococcin(50 mg/m2 plus 200 ng) injected 1 times per week for 4 consecutive weeks.Results The efficiency in the four groups was respectively 36.8%(group Ⅰ),41.6%(group Ⅱ),81.25%(group Ⅲ) and 89.4%(group Ⅳ).The efficiency in group Ⅲ was higher than that in group Ⅰ and groupⅡ with significant differences(P<0.01);The efficiency in group Ⅳ was higher than that in group Ⅰ and group Ⅱ with significant differences(P<0.01).There were no significant differences between group Ⅰand group Ⅱ(P>0.05),and group Ⅲ and group Ⅳ(P>0.05).Conclusion Four groups of drugs all had good effect on pleural effusion caused by lung cancer,while cisplatin plus staphylococcin was not superior to single staphylococcin.
