长期家庭氧疗对慢性阻塞性肺疾病合并慢性肺源性心脏病患者疗效观察

目的探讨长期家庭氧疗COPD合并慢性肺源性心脏病(简称肺心病)患者的疗效。方法将93例COPD合并肺心病的患者随机分为长期家庭氧疗组(LTDOT组,48例)和对照组(45例)。分别观察两组治疗前、治疗后1年和2年的超声心动图、周围血象变化。结果氧疗组1年及2年后的红细胞计数和红细胞压积比较治疗前均无明显变化。对照组2年后红细胞计数高于治疗前(P<0.05)。超声心动图示氧疗组患者1年后的肺动脉收缩压(PASP)较治疗前下降(P<0.05),2年后亦低于1年后(P<0.05)。结论长期家庭氧疗可降低COPD合并肺心病患者的肺动脉压,延缓肺心病的恶化。

Clinical observation of long term domiciliary oxygen therapy on Chronic obstructive pulmonary complicated with pulmonary heart disease patients

Objective To explore the effects of long term domiciliary oxygen therapy on chronic obstructive pulmonary complicated with pulmonary heart disease patients.Methods 93 COPD(chronic obstructive pulmonary disease) with heart disease patients were randomly divided into two groups,LTDOT group(n=48) and control group(n=45).The changes of Echocardiography and blood routine before treatment and 1 year and 2 year after treatment were carefully examined and recorded.Results There were no obvious changes in red blood cell counts and the level of hemoglobin between 1 year after treatment and before treatment.However,after 2 years,the red blood cell counts were higher than thosr before treatment(P<0.05).The Echocardiography showed that the PASP(Pulmonary Artery Systolic Pressure) of patients in LTDOT group decreased after treatment more significant ly than before treatment(P<0.05),and the PASP after 2 years was lower than that after 1 year(P<0.05).Conclusion Long term domiciliary oxygen therapy can help to reduce the pulmonary artery systolic pressure of COPD complicated with heart disease patients and postpone deterioration of pulmonary heart disease.
