CPAP短期治疗对OSAHS患者认知障碍及胰岛素样生长因子-1水平的影响

目的 观察持续气道正压通气(CPAP)短期治疗对阻塞型睡眠呼吸暂停低通气综合征(OSAHS)患者认知障碍及血清胰岛素样生长因子-1(IGF-1)水平的影响.方法 选择确诊为中重度OSAHS患者14例为研究组,15例健康志愿者作为对照组.两组患者均进行简易智能状态量表(MMSE)评分和血清IGF-1水平检测,研究组患者经过连续4天CPAP治疗后再次行MMSE评分及IGF-1水平检测.结果 研究组呼吸暂停低通气指数(AHI)明显高于正常对照组(P<0.01),MMSE总分、MMSE亚组(短时记忆、注意与计算、图形描画)分值、最低SaO2(SpO2LOW)、血清IGF-1水平明显低于对照组(P<0.01).14例研究组患者经过4晚CPAP治疗后,MMSE总分、MMSE亚组(短时记忆、注意与计算)分值及血清IGF-1水平明显升高(P<0.01).结论 CPAP短期治疗在防治OSAHS 引起的认知功能障碍方面有一定的作用.

Effects of short-term treatment of Continuous positive airway pressure on the Cognitive disorder, Serum Insulin-Like Growth Factor-1 in Patients With Obstructive Sleep Apnea hypopnea Syndrome

Objectives To observe the influence of short-term treatment of continuous positive airway pressure(CPAP) on cognitive function,serum insulin-like Growth Factor-1(IGF-1) level in the patients with obstructive sleep apnea hypopnea syndrome(OSAHS).Method The IGF-1 in serum of 14 patients with OSAHS both before and after the CPAP 4 days treatment were detected by means of enzyme immunoassay,and they were assessed for congnitive function using the Mini-Mental Status Examination(MMSE),and 15 normal controls were assayed at the same time.Results The values of IGF-1in serum in the OSAHS group was 256.1±76.4 ng/ml,and that in the control group was 318.4±86.1 ng/ml(P<0.01).The differences of AHI,SpO2LOW,the total score of MMSE were statistically significant between the two groups of the individuals(P<0.01).The values of IGF-1 in serum in the OSAHS group after 4 days CPAP treatment was 400.7±101.3 ng/ml,higher than that the in the OSAHS group(P<0.01).Conclusions CPAP short-term treatment can improve the IGF-1 level in serum,and be helpful for preventing cognitive disorder by OSAHS.
