松动人工髋关节界膜显微和超显微结构的研究

摘    要:目的 通过观察松动人工髋关节界膜的组成及其内在成分的显微和超显微结构来控制人工关节松动的生物学因素,方法 选择22例行人工全髋关节翻修术的患者,术中观察界膜的肉眼形态及人工假体的改变,取界膜送病理检查,有8例患者界膜行扫描和透射电镜检查,结果 界膜主要细胞成分处于激活状态,,巨噬细胞吞噬和包绕磨损颗粒,巨噬细胞和成纤维细胞粗面内质网丰富,高尔基复合体发达。结论 松动人工髋关节界膜的形成主要是机体对磨损颗粒反应的结果,界膜内巨噬细胞,成纤维细胞在人工关节的生物学因素中起重要作用,淋巴细胞的作用有待进一步研究。

Abstract:Objective To study the biological reason of loosening prostheses of hip by observing the interface membranes microstructure and ultra-microstructure. Methods Interface membrane around loosened prosthesis was harvested from 22 patients undergoing the revision of total hip replacement.All specimens were underwent light microscopic observation.The specimens of 8 were underwent electronic microscopic observation. Results The cells in interface membrane were active.Macrophages and multinuclear giant cells phagocytose and encyst the wear debris.Golgi complex and rough-surfaced endoplasmic reticulum of macrophages and fibroblast were rich. Conclusions The forming of interface membrane in loosening hip prostheses is the result of the body cells reacted to wear debris.The function of the lymphocyte in interface membrane should be studied more.
