黏蛋白MUC2、MUC5AC在卵巢上皮黏液性肿瘤中的表达及临床意义

目的 探讨黏蛋白MUC2、MUC5AC在原发性及结肠转移性卵巢上皮间质肿瘤组织中的表达规律及其临床意义。方法 采用免疫组化方法（SABC法），检测MUC2、MUC5AC在41例原发性卵巢黏液性肿瘤（良性12例、交界性9例、黏液性囊腺癌20例）、18例卵巢结肠转移性黏液性囊腺癌组织中的表达，并将MUC2、MUC5AC表达与临床各病理资料间的联系进行相关分析。结果 （1）18例卵巢的结肠转移性黏液性囊腺癌组织中均有MUC2表达，但无MUC5AC表达；（2）MUC2在良性、交界性、恶性原发性卵巢黏液性肿瘤组织中的阳性表达率分别为8．3％（1／12）、5／9、80．0％（16／20），MUC5AC阳性表达率分别为100．0％（12／12）、9／9、90．0％（18／20）；（3）MUC2阳性表达率与卵巢黏液性肿瘤的病理分级之间呈负相关（P〈0．05），与临床分期之间无相关关系（P〉0．05）：MUC5AC阳性表达率与卵巢黏液性肿瘤的临床分期、病理分级之间均无相关关系（P〉0．05）。结论 MUC2、MUC5AC黏蛋白可用于原发与卵巢结肠转移性黏液性肿瘤的诊断；随着卵巢黏液性肿瘤的发展在良性、交界性、恶性卵巢黏液性肿瘤组织中MUC5AC的阳性表达率呈下调表达；MUC2的阳性表达率呈上调表达，其表达与卵巢黏液性肿瘤的细胞病理分级密切相关。

The Expression and Clinical Significance of MUC2,MUC5AC Detection in Ovarian Epithelial Mucinous Neoplasma

Objective To investigate the expressive regularity and the clinical significance of MUC2,MUC5AC in primary and matastatic ovarian carcinomas. Methods The expression of MUC2,MUC5AC was detected by immunohistochemistry method (SABC) in 41 primary ovarian mucinous neoplasma (benign 12,borderline 9,adenocarcinoma 20),18 ovarian matastatic tumors which arise from colon,then the relationship between the expression and pathological index was analyzed. Results (1) The expression of MUC2 was detected in 18 ovarian matastatic tumors,but MUC5AC had no expression.(2) The positive rate of MUC2 expression in 41 primary ovarian tumors (benign,borderline,malignant) was 8.3% (1/12),5/9,80% (16/12) respectively,and the positive rate of MUC5AC in those carcinomas was 100% (12/12),9/9,90% (18/20).(3) There is negative relationship between the expression of MUC2 and pathological grade of ovarian malignant tumors (P<0.05),and there was no relation to clinical stage (P>0.05).There is no relationship between the expression of MUC5AC and clinical stage,pathological grade of the ovarian carcinomas (P>0.05).Conclusion MUC2 and MUC5AC genes may be two useful tumor markers in the diagnosis of primary and matastatic ovarian carcinoma.The expression of MUC5AC in ovarian carcinomas show down-regulation,but the expression of MUC2 in ovarian tumors show up-regulation,there is close relationship between the expression and pathological grade of ovarian carcinoma.
