内镜诊断原发性胃恶性淋巴瘤

目的 :为探讨原发性胃恶性淋巴瘤的临床表现、胃镜下特征 ,以提高对恶性淋巴瘤的认识 ,提高该病的胃镜诊断率。方法 :选取我院 10年间经手术后病理证实的 2 1例胃原发性恶性淋巴瘤 ,分析其胃镜下表现、临床特点及一般临床资料。结果 :胃原发性恶性淋巴瘤可发生于任何年龄 ;胃镜下缺乏特异性 ,表现为多行性、弥漫性及范围广的特征。内镜下难于同其他胃恶性肿瘤鉴别。结论 :原发性胃恶性淋巴瘤是临床上少见的胃恶性肿瘤 ,术前误诊率高 ,为提高诊断率 ,对胃镜下隆起性病变尤其是肿块表面伴溃疡时应高度怀疑恶性淋巴瘤 ,取病理时应尽量多取、深取 ,必要时重复活检。同时 ,镜检医生及病理医生应加强对该病的认识

Endoscopic diagnosis of primary malignant lymphoma of stomach

Objective The clinical characteristics and gastric endoscopic features were analysed in order to improve the recognizing and diagnostic levels about this disease.Method Choosing 21 patients with the primary malignant lymphoma of stomach which have been operated and verified by pathology in ten years,to analyses its clinical characteristics and endoscopic features.Results Primary malignant lymphoma can occurred in any ages and lacking of endoscopic characteristic,its endoscopic morphology looked more complicated,polymorphism and multicentrics,and it is very difficult to distinguished with other gastric malignant tumors by endoscope.Conclusion Primary gastric malignant lymphoma is very likely to be misdiagnosed or neglected due to is rarity and specificity.To improve its diagnostic levels and reduced its misdiagnosed rat,the endoscopic manifestation must be emphasized and the biopsy mast taken mostly and deeply,and the endoscopic pathologic doctors should improving the recognition of this disease.
