经皮椎间孔镜髓核摘除术治疗腰椎间盘突出症的疗效分析

目的观察经皮椎间孔镜技术治疗腰椎间盘突出症的近期疗效。方法我院于2013年6月~2017年3月采用经皮椎间孔镜下髓核摘除术治疗腰椎间盘突出症256例,应用疼痛视觉模拟评分(VAS评分)和改良Macnab标准评价其临床疗效。结果术后1周时、3个月时、1年时VAS评分低于术前,差异有统计学意义(P0.05)。根据改良Macnab标准评价术后1年时临床疗效:优192例,良32例,可28例,差4例,优良率为87.5%(224/256)。所有患者均无椎间隙感染、硬膜囊破裂、神经根损伤等并发症发生。结论经皮椎间孔镜技术治疗腰椎间盘突出症疗效显著,且具有操作安全、创伤小、术后恢复快等优势,严格掌握手术适应证是保证手术疗效的关键。

Efficacy analysis of percutaneous transforaminal endoscopic discectomy for the treatment of lumbar disc herniation

Objective To observe the short-term efficacy of percutaneous transforaminal endoscopic treatment of lumbar disc herniation.Methods 256 cases of lumbar intervertebral disc herniation treated with percutaneous transforaminal endoscopic discectomy in our hospital from June 2013 to March 2017 were selected.The clinical efficacy was evaluated by visual analog scale score(VAS)and modified Macnab criteria.Results The VAS scores at 1 week,3 months,and 1 year after surgery were lower than those before surgery,and the difference was statistically significant(P<0.05). According to the modified Macnab criteria,the clinical efficacy was evaluated at 1 year after surgery as follows:192 cases were excellent,32 cases were good,28 cases were effective,and 4 cases were poor,with the excellent and good rate was 87.5%(224/256).All patients had no complications such as intervertebral space infection,dural sac rupture,and nerve root injury.Conclusion Percutaneous transforaminal endoscopic treatment of lumbar disc herniation is effective,and has the advantages of safe operation,less trauma,quicker postoperative recovery,etc.Strict control of surgical indications is the key to ensure surgical outcome.
