超高频经直肠三维超声直肠癌局部分期诊断价值分析

目的对超高频经直肠三维超声直肠癌局部分期诊断价值进行分析及判定。方法选取2015年2月~2016年1月期间本院收治的30例直肠癌患者作为本次的研究对象,均行超高频经直肠三维超声诊断,并将其结果与手术病理结果予以对比,并对其诊断方案的准确性进行统计学分析。结果多数直肠壁肿瘤患者的肠壁肿瘤以浸润性为主要表现,部分表现为肿块。其经三维超声诊断的准确例数为29例,其准确率为96.67%,其PT1期4例,PT2期6例,PT3期15例,PT4期5例;u T1期3例,u T2期7例,u T3期16例,u T4期4例。u T1的诊断准确率、敏感度及特异性分别为96.67%、75.00%、100.00%;u T2的诊断准确率、敏感度及特异性分别为96.67%、100.00%及95.83%,u T3的准确率、敏感度及特异性分别为93.33%、93.75%及92.86%;u T4的准确率、敏感度及特异性分别为96.67%、100.00%及100.00%。结论超高频经直肠三维超声直肠癌局部分期诊断价值显著,且对于直肠癌局部的T分期的效果尤为显著,亦对淋巴结N分期的诊断价值具有一定的优越性。

Diagnostic value analysis of ultra high frequency transrectal three-dimensional ultrasound in local staging of colorectal cancer

Objective To analyze and determine the diagnostic value of ultra high frequency trransrectal three-dimensional ultrasonography in local staging.Methods 30 patients with rectal cancer admitted in our hospital from February 2015 to January 2016 were enrolled in this study.The patients all underwent ultra high frequency transrectal three-dimensional ultrasound.The ultrasound results were compared with surgical pathological results and the diagnostic accuracy of the diagnostic programs were statistically analyzed.Results The invasive intestinal wall tumors were the main performance for most patients with rectal wall tumors,with lump as the part performance.The accurate cases of threedimensional ultrasound diagnosis in rectal wall tumors were 29 cases,with accuracy rate of 96.67%,including PT1 in 4 cases,stage PT2 in 6 cases,stage PT3 in 15 cases,stage PT4 in 5 cases;stage uT1 in 3 cases,stage uT2 in 7 cases,stage uT3 in 16 cases and stage uT4 in 4 cases.The diagnostic accuracy,sensitivity and specificity of uT1 were 96.67%,75.00%and 100.00%respectively.The diagnostic accuracy,sensitivity and specificity of uT2 were 96.67%,100.00%and 95.83%.The accuracy,sensitivity and specificity of uT3 were 93.33%,93.75%and 92.86%respectively.The accuracy,sensitivity and specificity of uT4 were 96.67%,100.00%and 100.00%respectively.Conclusion The ultra-high frequency rectal three-dimensional ultrasonography has significant diagnostic value and analysis value in local staging,and its effect on local T staging of rectal cancer is particularly significant.It also has some advantages in the diagnosis of lymph node N staging.
