肝胆外科病人感染病原菌分布及耐药性研究

目的研究肝胆外科病人感染病原菌种类及其耐药性,为临床合理使用抗菌药物提供参考。方法采用细菌分离培养和药敏试验方法,对某医院肝胆外科病房1 200份送检标本进行细菌鉴定及药敏试验。结果从1 200份标本中共检出病原菌720株,检出率60%。在检出的病原菌中,革兰阴性杆菌占60%,其中肺炎克雷伯菌、大肠埃希菌和铜绿假单胞菌居前3位;革兰阳性球菌占40%,主要包括金黄色葡萄球菌、肠球菌和表皮葡萄球菌等。药敏试验表明,检出的病原菌普遍耐药,革兰阳性球菌只对万古霉素全部敏感,革兰阴性杆菌只对亚胺培南敏感。结论肝胆外科病人感染病原菌主要是革兰阴性杆菌,其次是革兰阳性球菌,这些病原菌普遍耐药。

DISTRIBUTION AND ANTIBIOTIC RESISTANCE OF PATHOGENIC BACTERIA ISOLATED FROM INFECTED PATIENTS IN HEPATOBILIARY SURGERY

Objective To study the class and antibiotic resistance of pathogenic bacteria isolated from infected patients in Hepatobiliary Surgery,so as to provide reference for clinic antimicrobial treatment.Methods A total of 1 200 samples were isolated to analyze the strain identification and drug sensitivity test.Results Of the 1 200 samples,720 strains pathogenic bacteria were detected out,the detection rate was 60%.60% were Gram-negative bacteria and 40% were Gram-positive cocci.The predominant pathogens of Gram-negative bacteria were Klebsiella pneumonia,Escherichia coli and Pseudomonas aeruginosa;the predominant pathogens of Gram-positive cocci were Staphylococcus aureus,Enterococcus and Staphylococcus epidermidis.The susceptibility testing showed that the pathogenic bacteria were generally resistant to antibiotic.Gram-positive cocci were only sensitive to Vancomycin,and Gram-negative bacilli were only sensitive to Imipenem.Conclusion The pathogenic bacteria isolated from hepatobiliary surgical infected patients were mainly Gram-negative bacteria,followed by Gram-positive cocci,and all these pathogens were generally resistant to antibiotics.
