摘    要:目的:通过高原红细胞增多症(HAPC)患者肺功能与正常对照组肺功能变化的比较,探讨高红症肺功能受损的原因及危险因素。方法:测定30例高红症患者的肺功能,并与正常对照组从年龄,性别,居住海拔高度,肺通气功能,弥散功能等方面进行比较。结果:两组在年龄、性别等方面无统计学差异。高红组患者肺功能各项通气指标(VC MAX、FVC、FEV1、FEV1%FVC、PEF、FEF25、FEF50、FEF75、MMEF75/25)及弥散指标(TLCO/VA)均降低;而残气量及残总比指标(RV-He、RV%TLC-He)增加。高红组吸烟者与对照组吸烟者比较MV指标显著降低。结论:高红症患者有明显的肺功能损害,主要表现为肺泡通气量明显降低和(或)弥散功能降低,而残气量有所增加。吸烟、体力劳动、长期生活的海拔高度有可能是HAPC的危险因素。

Abstract:Objective:To compare the changes of lung function between high altitude polycythemia(HAPC) patients and control group who have normal pulmonary function,and discuss the causes impairing lung function and the risk factors of HAPC.Methods:Lung function of 30 cases of HAPC,including VC MAX、FVC、FEV1、FEV1%FVC、PEF、FEF25、FEF50、FEF75 and MMEF75/25,and 30 controls(healthy adults) was measured using Jaeger type automatic pulmonary function tester.The assessment of age,sex,residence altitude,pulmonary ventilation and diffusing function,etc were conducted between the two groups.Results:Level of VC MAX、FVC、FEV1、FEV1%FVC、FEV1%VCMAX、FEV1*30、PEF、FEF25、FEF50、FEF75、 MMEF75/25,and diffusing index are decreased in the HAPC group,and the RV and RV%TLC-He index are increase.As compare with the smoking subgroup in control group,MV index was obvious decrease in the subgroup of HAPC group.Conclusions:In the patients with HAPC,there is significant impairment of pulmonary function,especially in decreased alveolus ventilation,diffusing function,and increased residual volume.Smoking,manual labor and increased altitude could be the risk factors causing HAPC.
