剖宫产后硬膜外镇痛对泌乳的影响及护理

摘    要:目的探讨剖宫产硬膜外镇痛对泌乳的影响及护理措施。方法 120例剖宫产产妇随机分为观察组和对照组各60例。观察组术毕给予硬膜外自控镇痛及相应护理,对照组采用肌内注射哌替啶100mg次/的术后镇痛方法及普通护理。观察2组初乳时间、24h哺乳次数、乳汁不足发生率及产后48h血清泌乳素分泌情况。结果观察组初乳时间短于对照组,24h哺乳次数和产后48h血清泌乳素分泌量多于对照组,24h乳汁分泌不足发生率为5%低于对照组的13.3%,差异均有统计学意义(P〈0.05)。结论剖宫产硬膜外镇痛结合相应护理可促进产妇泌乳。

Abstract:Objective To investigate the effect of epidural analgesia after cesarean section on lactation,as well as its nursing measures.Methods 120 cases of delivery woman underwent cesarean section were randomly divided into observation group and control group,60 cases of each.The observation group was given epidural patient controlled agalgesia(EPCA) and corresponding care after operation,while the control group was given pethidine 100mg per time im for relieving pain after operation and general care.The colostrum time,times of breast-feed,incidence rate of less milk in 24 hours and the situation of postpartum serum prolactin secretion in 48 hours of 2 groups were observed.Results The colostrum time of the observation group was shorter than that of the control group,with more breast-feed times in 24 hours and serum prolactin secretion in 48 hours,the incidence rate of less milk in 24 hours of the observation group was 5% lower than 13.3% of the control group,and all the differences were statistically significant(P0.05).Conclusion Cesarean section with epidural analgesia and proper care can accelerate lactating of postpartum.
