不同剂量纳洛酮治疗急性乙醇和吗啡混合性中毒昏迷疗效观察

目的观察不同剂量纳洛酮治疗乙醇和吗啡混合性中毒昏迷患者的临床效果。方法将103例乙醇和吗啡混合性中毒昏迷患者随机分成纳洛酮治疗1、2、3、4组和对照组。治疗1～4组分别加用纳洛酮0.4mg、1.0mg、2.0mg和5.0mg稀释到20ml后静脉推注,每2小时1次;对照组给予醒脑静注射液20ml用5%葡萄糖注射液稀释至250ml后静脉滴注。观察各组患者苏醒时间,治疗后0.5、1、2、4、8、12、24、48和72h格拉斯哥（GCS）评分。结果各组GCS评分比较差异无统计学意义（P〉0.05）;治疗各组完全苏醒时间均短于对照组,差异均有统计学意义（P〈0.05）。结论纳洛酮对乙醇和吗啡联合中毒所致的急性昏迷,具有较强催醒作用,在一定范围内存在剂量依赖关系。

Clinical study of different dose of naloxone in treatment of coma on acute intoxication of alcohol and morphine

Objective To study the effect of different dose of naloxone in treatment of coma on acute intoxication of alcohol and morphine.Methods 103 cases of patients suffered from intoxication of alcohol and morphine were randomly divided to naloxone treatment group of 1,2,3,4 and control group,the treatment group of 1~4 were given naloxone for 0.4mg,1.0mg,2.0mg and 5.0mg respectively,the control group received Xingnaojing 20ml with 5% glucose injection diluted to 250ml intravenous drip.Observed all the groups of GCS score and complete recovery time.Results GCS score of each group had no statistically significant difference（P〉0.05）;All of naloxone treatment groups of complete recovery time was shorter than the control group,the differences were statistically significant（P〈0.05）.Conclusion The therapeutic effect of naloxone for acute intoxication of hypnotic and morphine is significantly,and inacertain range of menory in adose-dependent relationship.
