2型糖尿病颈动脉粥样硬化与尿白蛋白排泄率的关系

目的探讨2型糖尿病尿白蛋白排泄率（UAER）与颈动脉粥样硬化的关系。方法96例2型糖尿病患者,采用免疫比浊法测定UAER。依据UAER高低将所有患者分为尿白蛋白正常组（NA组）66例,微量白蛋白尿组（MA组）15例,明显白蛋白尿组（OA组）15例。行颈动脉彩色多普勒检声查颈动脉内膜中层厚度（IMT）并计算硬化斑块指数（PI）。分析2型糖尿病患者颈动脉IMT异常及硬化斑块发生与UAER的关系,并比较3组临床资料。结果OA组低密度脂蛋白明显高于NA组,差异有统计学意义（P〈0.05）;2型糖尿病患者UAER水平与IMT、PI均呈显著正相关（r=0.573和r=0.303,P均〈0.01）。结论UAER升高可能增加2型糖尿病患者大血管病变的发生风险,应进行早期防治。

Relationship between urinary albumin excretion rate and carotid arterial atherosclerosis in type 2 diabetes mellitus

Objective To evaluate the relationship between carotid arterial atherosclerosis and urinary albumin excretion rate（UAER）in type 2 diabetes mellitus patients.Methods 96 cases of patients with type 2 diabetes mellitus were enrolled.UAER were measured with immunoturbidimetry,based on the level of UAER all the patients were divided into normal urinary albumin group of 66 cases（group NA）,micro-albumin group of 15 cases（group MA）,and 15 cases of overt albumin（group OA）.Carotid arterial intimal-medial thickness（IMT）were examined through color doppler ultrasound,and the sclerosis plaques index（PI）were calculated.The relationships among abnormal IMT、sclerosis plaques and UAER in type 2 diabetes mellitus patients were analysed and the action of UAER on macrovascular pathological changes in type 2 diabetes mellitus was evaluated.Compared clinical data of the three groups.Results The low-density lipoprotein of group OA was significantly higher than group NA,the difference was statistically significant（P〈0.05）;IMT、PI of group MA and group OA were also higher than group NA,the differencens were statistically significant（P〈0.05）;UAER was significantly positively correlated with IMT and PI in type 2 diabetes mellitus patients（r=0.573 and r=0.303,P〈0.01）.Conclusions The higher UAER may increase the risk of macrovascular pathological changes in the type 2 diabetic patients.It is important to prevent and treat the microalbumuria early in the patients with type 2 diabetes mellitus.
