攻下开窍方保留灌肠治疗肝性脑病患者的临床效果及对肠道黏膜屏障功能的影响

目的探讨攻下开窍方保留灌肠治疗肝性脑病(HE)患者的临床效果及对肠道黏膜屏障功能的影响。方法选取陕西中医药大学附属医院2016年6月至2020年6月收治的90例HE患者,按照入院顺序的奇偶性将其分为对照组和观察组,每组45例。对照组在常规西医治疗基础上给予乳果糖口服溶液,观察组在对照组基础上给予攻下开窍方保留灌肠。比较两组的临床效果。结果观察组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的ALT、AST、TBIL、ET、BA水平、L/M排泄率均降低,Occludin、Claudin-1水平均升高,且观察组优于对照组,差异具有统计学意义(P<0.05)。结论攻下开窍方保留灌肠联合常规西医疗法能显著提高HE患者的临床效果,保护肠黏膜屏障,值得临床推广应用。

Clinical effect of retention enema with Gongxia Kaiqiao recipe in the treatment of patients with hepatic encephalopathy and its influence on intestinal mucosal barrier function

Objective To explore the clinical effect of retention enema with Gongxia Kaiqiao recipe in the treatment of patients with hepatic encephalopathy(HE)and its influence on intestinal mucosal barrier function.Methods A total of 90 patients with HE admitted in the affiliated hospital of Shaanxi university of Chinese medicine from June 2016 to June 2020 were selected and divided into control group and observation group according to the parity of admission order,with 45 cases in each group.The control group was given lactulose oral solution on the basis of conventional western medicine treatment,and the observation group was given retention enema with Gongxia Kaiqiao recipe on the basis of the control group.The clinical effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of ALT,AST,TBIL,ET,BA and L/M excretion rate in the two groups decreased,while the levels of Occludin and Claudin-1 increased,and those in the observation group were better than the control group,and the differences were statistically significant(P<0.05).Conclusion Retention enema with Gongxia Kaiqiao recipe combined with conventional western medicine can significantly improve the clinical efficacy of patients with HE,and protect intestinal mucosal barrier,which is worthy of clinical promotion and application.
