| 摘 要： | 目的 探讨恶性梗阻性黄疸术前减黄的价值.方法 对2002年3月至2008年8月中国医科大学附属第一医院普通外科80例手术治疗的恶性梗阻性黄疸患者进行回顾性分析.比较减黄组与未减黄组术后并发症的发生情况,判断恶性梗阻性黄疸患者术前减黄的临床意义.结果 80例恶性梗阻性黄疸的患者中56例(平均血胆红素167.3 μmol/L)术前未行减黄,其中术后并发症的发生率为42.9%(24/56);24例(平均血胆红素323.7 μmol/L)术前进行了减黄,术后并发症的发生率为66.7%(16/24).减黄组与未减黄组术后并发症的发生率无统计学差异(P>0.05).结论 减黄组与未减黄组术后并发症的发生率无统计学差异可能是由于减黄组减黄后胆红素水平降至与未减黄组接近,因此降低了其手术并发症的发生率.我们主张对于血胆红素>170 μmol/L,全身状态差,合并胆管炎的患者进行术前减黄.
