胫骨远端外侧锁定加压钢板治疗胫骨Pilon骨折

目的对胫骨远端外侧锁定加压钢板（LCP）治疗胫骨Pilon骨折的临床疗效进行观察并探讨。方法取河北省邯郸市第一医院骨一科自2011年以来收治的胫骨Pilon骨折患者54例，其中27例患者采取胫骨远端外侧锁定加压钢板治疗，视为实验组；另外27例患者通过切开复位内固定治疗，视为对照组。通过对患者手术切口大小、手术流血量和术后软组织感染统计和记录来评定治疗效果。同时所有患者均获得随访，并通过随访记录对两组治疗效果采用Mazur评分系统进行踝关节功能评定。结果实验组在手术切口大小、手术流血量和术后软组织感染明显优于对照组；术后随访2年，所有患者无骨不连的情况发生，骨折部位全部愈合。实验组踝关节功能评定结果：优16例，良7例，可4例，优良率为85．2％；对照组踝关节功能评定结果：优10例，良8例，可9例，优良率为66．7％；且差异均具统计学意义（P〈0．05）。结论远端外侧LCP治疗是常见的治疗胫骨Pilon骨折的手术方法，其临床效果显著，值得进一步推广。

Analysis of lateral distal tibia locking compression plate in treatment of tibia Pilon fracture

Objective To observe the clinical curative effect the lateral distal tibia locking compression plate （LCP） in treatment of tibia Pilon fracture and to explore~ Methods Our hospital since 2011 were tibia Pilon fractures in patients with 54 cases, of which 27 patients take the lateral distal tibia locking compression plate fixation, as experimental group; the other 27 patients by open reduction and internal fixation, as control group~ Through the statistics and records of soft tissue infection in patients with operation incision size, amount of bleeding in operation and after operation to evaluate the effect of treatment~ At the same timeall the patients were followed up, and through the follow - up records were ankle joint function evaluation of the therapeutic effects of two groups according to Mazur score system. Results In the experimental group, the control group was significantly better than that of soft tissue infection operation incision size, amount of bleed- ing in operation and after operation, postoperative follow - up of 2 years, all patients without bone nonunion, fracture heal- ing. The experimental group of ankle joint function evaluation ： excellent in 16 cases, good in 7 cases ,4 cases, the excel- lent and good rate was 85.2%, the control group of ankle joint function evaluation ： excellent in 10 cases, good in 8 cases, 9 cases was excellent and good rate was 66. 7% , and the differences were statistically significant （ P 〈 0. 05 ） ~ Conclusion The distal lateral LCP therapy is a common method of operation treatment of tibia Pilon fracture, the clini- cal effect is significant, worthy of further promotion.
