米索前列醇联合芬太尼和丙泊酚用于无痛人工流产的临床探究

目的探讨米索前列醇联合芬太尼及丙泊酚用于无痛人工流产的麻醉效果。方法收集十堰市妇幼保健院妇产科2011年3月至2012年3月间入院并需进行人工流产手术的妇女8l例。使用随机法将上述所有患者分为两组，即对照组40例与观察组41例。两组患者均行医院常规人工流产手术，对照组患者术中采用丙泊酚行常规麻醉。而治疗组患者术中则采用给予米索前列醇联合芬太尼及丙泊酚联合用药进行麻醉。针对两组患者术中麻醉效果、清醒时间、苏醒时VAS评分以及手术出血量等资料进行对比分析。结果两组患者手术中麻醉效果对比差异，其观察组显效及总有效率显著高于对照组，且结果差异（P〈0．05）具有统计学意义。而两组患者清醒时间、苏醒时VAS评分以及手术出血量对比差异，观察组患者也明显优于对照组患者，且结果差异（P〈0．05）具有统计学意义。结论米索前列醇联合芬太尼及丙泊酚用于无痛人工流产具有显著的麻醉效果，且可以有效减轻患者术中疼痛及手术出血量，提高苏醒速度及苏醒质量。故此方法安全有效，值得临床推广。

Clinical research of misoprostol combined with fentanyl and propofol for painless abortion

Objective To investigate the anesthetic effect of misoprostol combined with fentanyl and propofol for painless abortion. Methods Hospital Obstetrics and Gynecology ,81 cases of women admitted to hospital between March 2011 to March 2012 and the need for a surgical abortion. All the patients were divided into two groups,namely the control group of 40 patients with the observation group ,41 cases using the random method. The two groups of patients underwent the hospital routine surgical abortion, the conventional anesthetic propofol in patients of the control group. The treatment group patients given misoprostol combined with fentanyl and propofol combination anesthesia. For the two groups of pa- tients, anesthetic effect,wake time, wake up when the VAS score and operative bleeding were analyzed. Results The the patients surgical anesthetic effect of contrast differences, the observation group markedly and the total efficiency were sig- nificantly higher than those the control group, and the results were statistically significant differences （ P 〈 0. 05 ）. Awake time of the two groups of patients, VAS score and surgical bleeding amount of contrast differences when waking the obser- vation group were significantly better than those the control group patients, and the results were statistically significant differences （ P 〈 0. 05 ）. Conclusion Misoprostol combined with fentanyl and propofol for painless abortion significant anesthetic effect, can effectively reduce the amount of pain and surgical bleeding in patients undergoing, to improve the speed of awakening and awakening quality. Therefore the method is safe and effective, and worthy of promotion.
